INFORMED CONSENT, RELEASE AND WAIVER

WAIVER, INDEMNITY
I am a willing participant in the Greater Sudbury Police Service Truth and Reconciliation Relay and participation is voluntary. I have been advised, understand, and agree that there are numerous risks inherent in and associated with participation in the Greater Sudbury Police Service Truth and Reconciliation Relay, which vary, and which risks range from 1) minor injuries such as scratches, bruises, and sprains; 2) major injuries such as eye injury, or loss of sight, joint or back injuries, heart attacks, and concussions; or 3) catastrophic injuries including paralysis and death.

I UNDERSTAND AND ACCEPT these and all other risks inherent to the participation Greater Sudbury Police Service Truth and Reconciliation Relay and I voluntarily assume the same. I further acknowledge that these risks cannot be prevented or avoided even by the exercise of reasonable care. I WARRANT that I am/the Participant is physically, mentally, and emotionally fit to participate in the Greater Sudbury Police Service Truth and Reconciliation Relay

I AGREE NOT TO HOLD RESPONSIBLE Greater Sudbury Police Service, Greater Sudbury Police Services Board, City of Greater Sudbury, their employees, agents, officials, contractors, representatives, elected and appointed officials, sponsors, servants, successors and assigns (individually or collectively called the “Released”) for any claims, demands, damages, costs, expenses, actions, injuries, losses, or damages whether in law or equity, sustained by me/us (including the Participant) or my property as a result of participation in the Greater Sudbury Police Service Truth and Reconciliation Relay , or any activities associated with the Greater Sudbury Police Service Truth and Reconciliation Relay, or as a result of travelling to and from the Greater Sudbury Police Service Truth and Reconciliation Relay , notwithstanding that same may have been contributed to or occasioned by the negligence of the Released.

I HEREBY RELEASE, WAIVE, AND FOREVER DISCHARGE the Released of and from any and all claims, demands, damages, costs, expenses, actions and causes of action whatsoever, whether in law or equity, in respect of death, bodily injury, personal injury, loss or damage (direct or indirect) to the Participant or myself, notwithstanding that same may have been contributed to or occasioned by the negligence of the Released.

I AGREE TO INDEMNIFY AND SAVE HARMLESS the Released from and against any and all liability related to participation in the Greater Sudbury Police Service Truth and Reconciliation Relay.

I FURTHER AGREE TO COMPLY AND ENSURE COMPLIANCE with any and all safety provisions and conditions imposed by the Released.

I DECLARE THAT I HAVE READ, UNDERSTOOD AND AGREE to the contents of the INFORMED CONSENT, RELEASE AND WAIVER form in its entirety.
